Need Utility Bill Design

I need someone who can make me a fake Bulgaria utility bill, print it out, and then take a picture of it for me to unlimit my PayPal account. The picture has to look like it is a REAL bill addressed to me (well my fake name), and addressed to me (well my fake address ).
